Thank you for entering your profile for the Talent Showcase.

The Talent Showcase is a software application being licensed by Team Adhoc for the purposes of promoting job candidates to employers.

By submitting your profile information through this self-entry form, you agree to the following:

• The information you have entered is accurate.
• Team Adhoc has the right to check references and verify any of the information you provided in the Talent Showcase self-entry form.
• Team Adhoc has the right to display the profile information your provided on the Talent Showcase, and to use any of the information you provided in email and other marketing to employers.
• Team Adhoc is not guaranteeing that you will appear on the Talent Showcase or that you will receive an offer of employment as a result of being included on the Talent Showcase.

IMAGE RIGHTS AND USAGE OPTIONS

You are required to indicate whether you consent to the use of this material by selecting one of the four confidentiality options presented on the form, and Team Adhoc will not use this material without your consent. Your consent and release will be kept on file. You may change or revoke your consent at any time.

Your confidentiality options are as follows:

1. Both name and photo - Team Adhoc has my permission to use my photograph or image along with a biographical description of my skills and experience for marketing purposes. I understand that this material may appear on the Team Adhoc website, in videos, podcasts and webcasts and in any online, printed or published materials such as brochures, email promotions, newsletters, and others.
2. Name ONLY - Team Adhoc has my permission to use a biographical description of my skills and experience for marketing purposes, but not my photograph or image. I understand that this material may appear on the Team Adhoc website, in videos, podcasts and webcasts and in any online, printed or published materials such as brochures, email promotions, newsletters, and others.
3. Photo ONLY - Team Adhoc has my permission to use my photograph or image along with a biographical description of my skills and experience for marketing purposes, but not my name. I understand that this material may appear on the Team Adhoc website, in videos, podcasts and webcasts and in any online, printed or published materials such as brochures, email promotions, newsletters, and others.
4. Neither name nor photo - Team Adhoc has my permission to use a biographical description of my skills and experience for marketing purposes, but not my name, photograph or image. I understand that this material may appear on the Team Adhoc website, in videos, podcasts and webcasts and in any online, printed or published materials such as brochures, email promotions, newsletters, and others.

PERMISSION TERMS

By submitting this form, I agree to the following:

1. SCOPE OF RIGHTS. I hereby grant to Team Adhoc the unrestricted right and permission to copyright and use photographic portraits of me and a biographical description of my skills and experience (the "Material"). Team Adhoc has the right to reproduce, exhibit, distribute, broadcast, digitize, edit, or otherwise use the Material, by any method and in any media, whether now existing or later created, without restriction throughout the world, by incorporating the Material into publications, catalogues, brochures, books, magazines, or commercial, informational, educational, advertising, or promotional materials, both online and print (collectively, the "Works"). I agree to cooperate with Team Adhoc regarding documenting or registering its rights to the Material. Team Adhoc shall have full control over all applications for such legal protection.
2. CERTIFICATION OF COPYRIGHT. I agree that Team Adhoc is and shall be the exclusive owner of all right, title, and interest, including copyright, in the Material and the Works.
3. WAIVER. I hereby relinquish and waive any right that I may have to examine or approve the Material and Works and waive any claim with respect to the eventual use of the Material.
4. GOVERNING LAW AND VENUE. This Agreement shall be governed in all respects by the laws of the State of CA without regard to conflict of law principles and venue shall be in CA.

By the submission of this form, I am accepting these terms on the Talent Showcase self-entry form. I grant my full and unqualified consent to the Permission Terms above.
